Before we can send your login, we need to verify your documents.  Read below to find your category of listing and send along the requested items.

Upon approval we will email your login invitation (less than 24 hours).

*If this is a renewal or if we already have your documents, you can skip this step. We will publish your profile shortly.

Required Documents:

Registered Applicants (non-students) – Email the following :
1. Your full name and a brief introduction of your practice

2. Image of your registration certificate from your Regulatory College
3. A photo for use on your profile (pre-cropped headshot, or we can crop it for you)  Please send a clear photo at least 250px by 325px

* In some cases we may also require a copy of your Diploma/Degree.

*Note – if your subscription was set up under a clinic name, on behalf of someone else, or using any name other than your own, please let us know these details in your email.

Student Applicants – Email the following:
1. Your full name and a brief introduction of your practice
2. The name, registration number, email address and/or website of your supervisor
3. Proof of insurance (omit if you are registered with a Regulatory College)
4. If applicable, a copy of your registration certificate from your Regulatory College
5. A photo for use on your profile (pre-cropped headshot, or we can crop it for you Please send a clear photo at least 250px by 325px

Practitioners in Unregulated Provinces – Email the following:
1. Your full name and a brief description of your practice, training, and credentials
2. Relevant certificates, diplomas, and association memberships
3. Where relevant, the name and email of your supervisor
4. Proof of insurance (mandatory)
5. A photo for use on your profile (pre-cropped headshot, or we can crop it for you) Please send a clear photo at least 250px by 325px
